Disclosing corona diagnosis to patients!

The second wave of the Corona virus pandemic in Tunisia is deadlier (than the first one) thanks to our politicians who do not know how to take the appropriate decisions. However, the elite of the country is not any better. The word elite refers to those professionals who spent decades being educated to become physicians.

Emergency rooms (ER) are flooded with patients infected with Corona Virus. Medical personnel are highly exposed to infectious risk (the number of infected health care professionals and especially those working in ER attests to that).

ER Physicians chose not to disclose the diagnosis of Covid 19 infection to patients in order not hurt their feelings. When you examine such position, you realize that it is wrong from the human, professional legal, practical and ethical point of view. These are the reasons:

1- The Tunisian law of July 27^th 1992 governing infectious and transmittable diseases requires Tunisian Physicians to disclose the diagnosis of transmissible diseases to infected patients.

2-There needs to be a discussion about the diagnosis with patients in order to create a trusting relationship between both parties. Otherwise patients will feel cheated and no longer trust physicians and the system.

3-Patient needs to be informed so he and or she can take appropriate precautions (as well as their family and entourage) in order to limit the progression of infection.

4-Informing patients will help slow down the infection rate.

5-If the infection rate is slowed down, theoretically the number of patients needing ER visits would decrease which means less work for those same physicians who chose not to disclose the diagnosis to their patients. There is no need to sacrifice physicians (and the reminder of the medical team) while exposing them to an avoidable infectious risk.

Here are 3 situations that results from not disclosing the diagnosis to patients:

a- Patient is being isolated and visits are prohibited while no reasons are given. How to explain such isolation and prohibition of visits to these individuals? This means that the patient is placed in a corner and no one talks to them. They are technically treated like objects denuded of all rights. This instance is not worthy of any human being. I was a witness of such abuses in the ER.

 

b- Visitors are allowed. These visitors are exposed to infections that could be deadly in a country with limited resources. I was witness of instances in which families were allowed to visit patients in the ER. These individuals would spend considerable time with their relative and other infected patients in the same room.

c- Patients and visitors are informed of the diagnosis. Visits are allowed. In this case doctors are willful participants in the spread of the disease.

All the situations mentioned above are illegal and against medical mission.

In other words, these physicians plunge themselves in deeper shit by: creating a flood of infected patients, risking infections and death themselves while practicing illegal actions. This is the highest level of intelligence.

Since, the Tunisian medical system has not finished showing me how disgraceful it is, it insisted on keeping me an extra 6 months for a do-over non-paid rotation in Tunisian hospitals. The goal of imposing such a supposedly punishment is to see with my own eyes abuses and report on them nationally and internationally. I will witness and report diligently. While in Italy, China, Spain, the UK, North America, physicians are doing extra efforts to insure the global welfare of patients and their families and society as whole, in Tunisia physicians do not respect the law and do not follow any national or international standards. What is the goal ultimately? Where is this taking us to?

La deuxième vague de la pandemie au coronavirus est plus meurtrière en Tunisie grâce à nos politiciens qui ne savent pas prendre les bonnes décisions. Cependant notre élite n`est pas meilleure. Et par élite,  je veux dire les professionnels qui ont passé des décennies à se former pour devenir médecins.

Les urgences sont submerges de patients infectes au Covid 19. Le personnel médical est des plus exposés au risque infectieux (le nombre de personnels de la sante et surtout les urgentistes infecté en Tunisie en témoigne).

Les urgentistes ont choisi de ne pas divulguer le diagnostic d`infection au corona virus aux patients soit disant ils ne veulent pas léser leurs sentiments. Sur le plan professionnel, humain, éthique, légal et pratique, cette attitude est fausse pour plusieurs raisons:

1- La loi tunisienne promulguée le 27/07/1992  régissant les maladies infectieuses et transmissibles oblige  le médecin Tunisien a divulguer le diagnostic de maladies infectieuses transmissibles au patient. 

2-Il faut discuter le diagnostic avec le patient pour que le patient ait confidence en ces médecins qui le traitent. Sinon c`est la crise de confiance. Le patient va se sentir triché.

3-Le patient doit être informe pour que lui, sa famille et son entourage prennent leur précautions pour ne pas s`infecter et ne pas infecter les autres.

4-Informer le patient aidera à freiner la progression de la pandémie.

5-Si on freine la pandémie, théoriquement le nombre de patients nécessitant des visites aux urgences sera contrôlé ce qui veut dire moins de travail pour ces mêmes urgentistes qui cachent le diagnostic a leur patients. Pourquoi sacrifier les médecins (et le reste de l`équipe médicale) en les exposant a un risque infectieux évitable?

Lorsque le diagnostic n`est pas divulgué au patient, il ya 3 instances qui se créent: 

a- Le patient est mis en isolation et on lui interdit toute visite. Comment explique-t-on cet isolation et cette interdiction des visites? Autrement dit on met le patient dans un coin et on le lui adresse pas la parole et on le traite comme un objet dénude de tout droit. Je ne trouve pas cette attitude digne d`un être humain et j`ai été témoin de telles dérives.

b- On permet les visites à ces patients et on expose les visiteurs au risque infectieux à la possible mort dans un pays à moyens très réduits. J`ai été témoin de familles de patients leur rendant visite et passant beaucoup de temps avec les malades dans des chambres pleines d`autres patients infectes.

c- Le patient et ses visiteurs sont informes du diagnostic et les visites sont permises. Le médecin dans ce cas consciemment favorise l`infection.

Toutes les instances citées ci-dessus sont illégales et contre la mission médicale.

Donc en fin de compte, on se fait enfoncer dans la merde de plus en plus: en se créant un déluge de patients infecte, en risquant l`infection et la mort soi-même et en étant hors la loi. C`est le summum de l`intelligence.

Comme on n`a pas encore fini de me montrer toute la disgrâce du système de sante Tunisien, on a insisté a ce que je refasse 6 mois de stage non payé pendant cette pandémie dans les hopitaux de la Tunisie. Le but d`imposer cette soit disant punition est de voir de mes propres yeux et reporter ces derives sur le plan nationl et international. Je le ferai avec diligence. Alors qu`en Italie, Chine, Espagne, Grande Bretagne, Amérique du Nord, les médecins sont entrain de faire un travail colossal pour assurer le bien être global des patients, de leur familles et de la société; nous en Tunisie, on ne respecte pas loi et on ne s`aligne a aucun critère national ou international. Dans quel but? Ou va-t-on?
