Nurses getting infected.

 Recently, I came to observe a strange phenomenon in Tunisian hospitals. After their shift work, nurses assigned to Covid unit remove their personal protective equipments and get together at the cafe next to the hospital. They sit tightly next to each other without any precautions: no social distancing and no mask wearing. It is winter, and in order to keep warm they sit near each other. It is under the watch of these nurses that Covid patients are experiencing the toughest predicaments. The sickest patients get admitted to the hospital. Each hospitalized patient needs to be isolated, needs oxygen and needs intravenous medications. Some overcome the viral infection and some die. If these nurses are so much involved with direct contact with these patients and are so much aware of these patients suffering and cannot comprehend the risk they are taking, there must be a problem. Worldwide, viral transmission within health care professionals (HCP) is high. It is the nature of the job: HCP are more exposed to the virus than any other profession. Tunisian HCP infection rate due to corona virus is among the highest in the world. HCP can be vectors of the disease (to family, friends with whom they spend time in cafes, patients and colleagues). They also can be contaminated from any source in these cafes. It seems like they are purposefully looking to get infected.

If these HCP intentionally get infected to have a sick leave, they are making serious calculation errors. They do not have any idea whether or not they would overcome the infection. Additionally, they can lose loved ones (mother, father, spouse, kid) that no sick leave or money can replace. Apparently, Tunisian nurses cannot sacrifice social gatherings during the pandemic (for more or less one year) for their own good and the good of their loved ones.

Tunisian nurses forget that if they facilitate virus transmission, le number of patients they need to care for increases.  

One of the reasons, nurse behave this way is the lack of educational support dedicated to them. People in charge (whether they are nurses, physicians or administrators from the ministry of health) do not even think about the need of continued nursing education. There are no programs supporting the endeavor of instilling good hygiene rules to nurses.

Another reason is; nurses do not have any role model to follow. They work with physicians practicing with no consciousness and watch managers (administrators or politicians) making corrupt decisions.  These mangers are never held accountable for their actions.

Anyways I keep saying that numbers do not lie. The mediocre Tunisian public health care performance in this pandemic is the consequence of such failures. Every group is watching the other to act first. Immoral physicians are waiting for a corrupt government. Unconscious nurses are waiting for rotten physicians.  As long as someone (a nurse or a physician or an administrator or a politician) is receiving a salary from the government he/she becomes a corrupt individual with no ethics. A new program gets downloaded to their brains. Do not expect any creativity from these civil servants. Talking about patriotism (serving the country who pays the salary) becomes old fashioned and obsolete.

I doubt that foreign countries will take on all Tunisians. In my opinion there will be always a limit to how many immigrants they would accept.

The vaccine is available; however the Tunisian government is not planning any vaccination effort before March/April 2021. Few months are needed before any kind of immunity is detected after vaccination. This means that the infection and the death toll are going to rise while waiting for the golden remedy. What are these nurses counting on while taking such unnecessary risks? Are they counting on a government paying them a salary to cloud their morality but does not provide a vaccine?

Il ya un phénomène que je viens d`observer dans les hôpitaux Tunisiens.

Après leur travail posté, les infirmiers affectés a l`unité Covid enlèvent leur tenue de protection personnelle et se dirigent vers le café a cote de l`hôpital. On s`assoit l`un à coté de l`autre sans aucune distanciation sociale et sans porter des masques. C`est l`hiver donc pour se maintenir au chaud on se met très proche l`un de l`autre.

Les infirmiers, qui s`occupent de patients infectés du Covid, sont au courant du périple dans le quel est engagé ces malades. C`est les patients qui sont les plus affectés qui ont besoin d`un séjour hospitalier. Tous patient hospitalisé  doit faire la quarantaine, tous ont besoin d`oxygène, tous sont mis sous des médicaments par voie intraveineuse. Certain survivent le virus et d`autres meurent. Si vraiment on est en contact aussi direct avec les patients et on est aussi averti de leur souffrance et on n`arrive pas a comprendre le risque qu`on prend, c`est qu`il y a un problème.

La contagion dans le milieu médical est élevée c`est la nature du travail : on est plus exposé que toutes les autres professions.  La contagion parmi le personnel de sante Tunisien est des plus élevée dans le monde. On peut être aussi bien un vecteur de la maladie pour les autres (sa propre famille, les camarades avec qui on s`assoit dans le café, patients et collègues dans l`hôpital) que être contaminé par les autres dans le café. C`est comme si on s`expose pour s`infecter exprès.  

Si on s`infecte exprès pour prendre un congé de maladie, on fait vraiment de faux calculs car on ne sait pas si on va surmonter l`infection. Aussi dans la foulée, on peut perdre des êtres chers (mère, père, conjoint, enfant) qu`aucun argent et aucun congé ne va remplacer

Il se trouve que les infirmiers Tunisiens ne peuvent pas sacrifier les rassemblements sociaux pendant une année pour le bien de soi même et pour le bien des autres.

Les infirmiers Tunisiens  oublient que s`ils transmettent le virus, le nombre de patients qu`ils vont voir va augmenter.

Une des raisons que les infirmiers se comportent de cette manière est le manque total d`encadrement. Les responsables que se soit des infirmiers, des médecins ou des administrateurs du ministère de la sante publique ne s`occupent pas de la formation continue des infirmiers. Il n`ya aucun programmes pour inculquer les bonnes règles d`hygiène aux infirmiers.

Une autre raison est que les infirmiers n`ont pas d`exemple à suivre. Ils observent des médecins sans conscience avec eux au travail et des responsables corrompus au pouvoir.

Les responsables ne sont pas redevables. On ne leur demande pas des comptes.

 

De toutes les manières (je le dis et redis) les chiffres ne mentent pas. La mauvaise performance des Tunisiens pendant cette pandémie est le résultat de défaillances pareilles. Chaque groupe attend l`autre groupe d`agir. Les médecins amoraux attendent le gouvernement corrompu et les infirmiers inconscients attendent les médecins pourris. Du moment où on reçoit un salaire du gouvernement on devient un vendu, un corrompu et on perd la morale. On ne peut attendre aucune initiative de ces fonctionnaires. Parler de patriotisme (servir le pays qui paye le salaire) devient démodé et périmé. Je ne crois que les pays étrangers vont prendre tous les Tunisiens. De mon avis il ya toujours une limite.

 

Le vaccin est la mais le gouvernement Tunisien a annonce que les efforts de vaccination ne vont pas commencer avant le mois de Mars/Avril. Il faut quelques mois pour faire monter une immunité. Sur quoi comptent ces infirmiers sur un gouvernement corrompu qui donne un salaire pour acheter les consciences mais qui ne fournit pas de vaccin ?
